Requirements

  • CASAC or CASAC-T preferred, or the ability to obtain within 6 months of hire.
  • Ability to deliver culturally competent services to a diverse client population in an urban setting.
  • Proficiency with computer operation (Microsoft Word, Excel, Electronic Health Records, and Outlook programs).
  • Must be able to work a flexible schedule.

Responsibilities

  • Providing counseling and case management services related to substance abuse, legal, mental health, medical and vocational/educational issues.
  • Documenting all provided services in a manner that is compliant with regulatory and third-party payor requirements.
  • Coordinating and providing treatment services based on the individualized treatment plan including discharge planning.
  • Advocating for and promoting a recovery-based lifestyle.
  • Working closely with the rest of the staff as an integral part of the clinical team.
  • Administering comprehensive psychosocial assessment tools at intake and at various points in accordance with Odyssey House policy and procedures.
  • Providing individual counseling, group counseling and case management services onsite and in the home or community.
  • Formulating a comprehensive individualized treatment plan in collaboration with each client, monitoring achievement of objectives, and reviewing and updating the treatment plan in accordance with clinical needs of the client.
  • Maintaining up-to-date client clinical records including at minimum: treatment planning, group counseling notes, progress notes, and the monitoring of social determinants of health in accordance with regulatory guidelines and program standards.
  • Providing crisis intervention services.
  • Responsible for ongoing client discharge planning beginning at admission.
  • Providing ongoing assessment of each assigned client, monitoring for any changes in clinical needs.
  • Assuring timely and effective care coordination with collateral services providers, family members and criminal/family justice agencies.
  • Delivering clinical services in accordance with evidence-based curricula and training.
  • Acting as a liaison with all referral agencies providing regular verbal and written progress reports.
  • Establishing community and agency affiliations which address the client’s need and strengthen the delivery of services.
  • Providing education and direct services to family members, as appropriate, regarding client’s substance use/misuse and related behavioral issues.
  • Conducting collection of samples for toxicology randomly and as needed.
  • Attending regularly scheduled staff meetings and case conferences.
  • Attending all required in-service training seminars.
  • Participating in quality improvement activities.
