About The Position

Requirements

  • Bachelor's degree in Psychology, Social Work, Counseling, or a related behavioral health field required.
  • Valid California Driver's License with an acceptable driving record.
  • Must be insurable under the agency's automobile insurance policy.
  • Successful completion of a Live Scan background check is required.
  • Proficiency with electronic health records and basic computer applications.
  • Ability to work independently while collaborating effectively within a multidisciplinary team.

Nice To Haves

  • Experience working with adults experiencing serious mental illness preferred.
  • Experience with Los Angeles County Department of Mental Health (LAC-DMH) documentation and procedures is highly preferred.
  • Military-experienced candidates are strongly encouraged to apply.
  • Bilingual (Spanish/English) is preferred but not required.

Responsibilities

  • Coordinate medical follow-up appointments and support clients with co-occurring medical and behavioral health needs.
  • Provide life skills training that promotes independence, wellness, and long-term recovery.
  • Assist clients in securing and maintaining stable housing by identifying and coordinating appropriate housing resources.
  • Connect clients with community-based services, benefits, and supportive resources to address individual needs.
  • Provide crisis intervention and stabilization services while supporting clients through challenging situations.
  • Complete timely, accurate, and thorough documentation in accordance with Los Angeles County Department of Mental Health (LAC-DMH), Medi-Cal, and agency standards.
  • Maintain required productivity expectations while ensuring compliance with all applicable regulations and documentation requirements.
  • Maintain confidential client records and support quality assurance efforts.
  • Work collaboratively within a multidisciplinary treatment team to coordinate comprehensive client care.
  • Participate in team meetings, case conferences, and treatment planning.
  • Follow established agency policies, procedures, and clinical protocols while managing an independent caseload.
  • Support continuous quality improvement initiatives and contribute to the success of the outpatient program.
