Requirements

• LMSW or higher licensure required, LCSW preferred
• Ability to demonstrate effective written and verbal communication skills and possess emotional and professional maturity
• Basic understanding of Medicare, Medicaid, and Managed Care regulations
• Organized and efficient in establishing priorities and performing tasks within specific timeframes
• Self-motivated requires minimal supervision
• Views problems as a challenge and investigates alternatives which demand creativity, flexibility, and the ability to work under stress
• Responsible for maintaining strict confidentiality of all verbal & written information and documents
• Familiarity with and appropriate use of community resources

Nice To Haves

• Three (3) or more years hospital social work or protective services experience preferred
• Experience in psychosocial and therapeutic counseling preferred
• Working knowledge/experience in case management, payors and reimbursement

Responsibilities

• Intervenes with patients who have complex psychosocial needs.
• Requires assistance with eligibility determination for social programs and funding sources.
• Qualifies for community assistance from a variety of special funds and agencies.
• Offers crisis intervention to patients and families with psychosocial needs.
• Helps coordinate a plan of care for high-risk patient populations.
• Receives referrals for individuals from at-risk populations from interdisciplinary team members.
