Family Health Care is a Federally Qualified Health Center (FQHC) committed to improving access to high-quality healthcare throughout rural Michigan. As a Clinic-Based Behavioral Health Therapist, you will provide assessment, diagnosis, treatment planning, psychotherapy, crisis intervention, and care coordination for individuals experiencing mental health, substance use, and co-occurring disorders. LLMSW and LLPC clinicians are encouraged to apply. In-house supervision is available for providers pursuing full licensure.

Requirements

Master's degree from an accredited program in: Social Work, Counseling, Psychology, Marriage and Family Therapy, or Related behavioral health field
Michigan licensure as one of the following: LMSW or LLMSW, LPC or LLPC, LMFT, LLP
Demonstrated knowledge and proficiency in behavioral health assessment and treatment.
Experience working with mental health conditions, substance use disorders, and co-occurring disorders.
Strong communication, clinical, organizational, and interpersonal skills.

Nice To Haves

One to three years of experience providing outpatient behavioral health services in settings such as outpatient therapy, community mental health, integrated healthcare, substance use disorder treatment, or Federally Qualified Health Centers (FQHCs).
Experience providing individual, family, and group therapy.
Training or experience in Cognitive Behavioral Therapy (CBT), Motivational Interviewing (MI), SBIRT, trauma-informed care, and recovery-oriented treatment approaches.
CAADC credential or willingness to pursue a CAADC Development Plan upon hire.
Applications from both experienced clinicians and limited-license professionals are encouraged.
In-house supervision is available for LLMSW and LLPC providers pursuing full licensure.

Responsibilities

Provide outpatient behavioral health counseling and psychotherapy services for individuals experiencing mental health, substance use, and co-occurring disorders.
Conduct comprehensive behavioral health assessments, diagnostic evaluations, treatment planning, and ongoing clinical interventions.
Deliver evidence-based individual, family, and group therapy services.
Utilize evidence-based practices including Cognitive Behavioral Therapy (CBT), Motivational Interviewing (MI), trauma-informed care, relapse prevention, and other clinically appropriate interventions.
Provide crisis intervention, risk assessment, care coordination, and referral services as needed.
Collaborate with primary care providers, MAT prescribers, behavioral health colleagues, and community resources to support coordinated patient care.
Support patients through treatment engagement, recovery planning, symptom management, and goal attainment.
Maintain timely, accurate, and compliant documentation within the electronic medical record (EMR).
Participate in quality improvement initiatives and organizational activities that support positive patient outcomes.
