This position performs injury assessments and refers patients for necessary care. The role involves communicating with patients to triage, answer questions, and instruct in self-care, prevention, and treatment. It also requires communication with the Physician and medical team to help determine the need for special tests or services. The individual will establish a plan of care and implement individual treatment based on evaluation results, health goals, and by utilizing appropriate techniques. They will communicate the plan of care to all involved in the patient's care, monitor and evaluate outcomes, and appropriately adjust the individualized treatment plan based on the patient's response to treatment. The role also involves identifying factors affecting patient's physical and psychosocial comfort and attending to these needs, with the ability to work with a diverse patient population. Planning for and completing patient after-visit summaries, including providing written home exercise programs, patient/family teaching, referral for community services, and ordering equipment, are key responsibilities. Completing all required documentation following department policy, including patient assessment, patient progress notes, and after-visit summaries as appropriate, is essential. The role also includes performing various patient positioning, transferring, and transporting duties, which require lifting and pushing/pulling, while utilizing proper technique. The incumbent must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served, understanding the principles of growth and development over the life span and possessing the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures.
