About The Position

Coordinating care between numerous healthcare providers Referring patients to providers who can assist them Sharing health and wellness tips relevant to patients Advocate for patients attempting to make use of public health and government programs Explaining health information in a culturally sensitive, easy-to-understand manner Leading support groups, workshops and seminars on topics of interest to the community Maintaining accurate records of all patient contacts Protecting patient confidentiality Advise clients or community groups on issues related to improving general health, such as diet or exercise Attend community outreaches to distribute flyers, brochures, or other informational or educational documents to perform outreach and awareness of the Department of Health programs Conduct home visits for targeted population clients to monitor their progress or assess their needs for resources in the paramedicine program

Requirements

  • Competent in Microsoft Office and assorted software
  • Competent in the use of an EMR for “real time” documentation
  • Competent in the use of equipment and supplies related to the position
  • Ability to provide Patient/Family Centered Care and resources
  • Ability to plan, organize and coordinate work assignments
  • Ability to communicate effectively
  • Ability to establish and maintain effective working relationships with others
  • High School Diploma
  • Valid Driver’s License
  • Must be able to bend, squat, or stand for prolonged time, and lift items up to 50 pounds

Nice To Haves

  • Bilingual in Spanish and English
  • Community Health Worker certification
  • Healthcare certifications/licenses or background experience
