About The Position

The Case Manager provides comprehensive case management tracking and follows up with the patient regarding their services. This includes intake assessments, benefit assessments, and booking referrals. The Case Manager also conducts research and reviews patient files to identify social or medical deterrents to achieving good health. Independently manages a patient caseload. Works directly with clinical staff and outside facilities to provide appropriate case management to include callback lists, postcards, and follow-up on referral statuses and outcomes. Communicates referral information to patients, referral agencies, and OneWorld providers as appropriate. Provides the patient with written instructions on how to navigate their appointment. e.g., appointment time, location, floor, procedure, pre-op instructions, interpreter services, etc. Issues written discharge instructions to patients and confirms follow-up appointments. Serves as a subject matter expert with agency partners (Hope, Every Women Matters, etc.), ensuring program compliance. Updates the treatment team on changes as necessary. Requirements

  • High School Diploma or equivalent required.
  • One or more years of healthcare experience required, preferably in the area of case management or patient care coordination.
  • Proficiency and experience with Microsoft Office products (Word, Excel, PowerPoint) required.
  • Proficiency in both English and Spanish required.
  • Demonstrated ability to communicate effectively with both internal and external customers.
  • Demonstrated organizational skills, with a high attention to detail and the ability to prioritize.
  • Demonstrated ability to analyze and solve problems.
  • Demonstrated ability to self-start and work independently, but also to function well in a team environment, all while exhibiting a high level of integrity and a sense of urgency to serve the patient.
  • Demonstrated understanding of the needs of low-income and medically underserved populations, and an awareness of the community resources available to them.

  • Bachelor’s Degree in Health Administration or a related field preferred.
