About The Position

Join a compassionate oncology team dedicated to supporting patients and families through the cancer care journey. As an Oncology Social Worker, you will provide counseling, advocacy, crisis intervention, and resource coordination while partnering with interdisciplinary teams to improve patient outcomes and quality of life.

Requirements

  • Master’s Degree in Social Work (MSW) from a CSWE-accredited program required.
  • Must maintain applicable state licensure requirements.
  • Knowledge of psychosocial assessment, crisis intervention, care coordination, and community resources.
  • Proficiency with electronic medical records and data management tools.
  • Commitment to the NASW Code of Ethics.

Nice To Haves

  • Licensed Clinical Social Worker (LCSW) preferred.
  • One year of hospital, oncology, or healthcare social work experience preferred.

Responsibilities

  • Conduct psychosocial assessments for oncology patients and their support systems.
  • Provide counseling, education, emotional support, and crisis intervention.
  • Develop and implement care plans with patients, families, and the multidisciplinary care team.
  • Facilitate discharge planning and transitions across the care continuum.
  • Connect patients and families with community, financial, and supportive care resources.
  • Advocate for patients by identifying and addressing psychosocial barriers to care.
  • Participate in multidisciplinary care planning and case review meetings.
  • Serve as a resource to staff regarding psychosocial aspects of illness and treatment.
  • Report suspected abuse, neglect, or exploitation in accordance with policy and regulations.
  • Maintain accurate documentation and medical record compliance.
