The Oncology RN Navigator coordinates care for patients preparing to start treatment, including pre-treatment labs, imaging, referrals, venous access placement, and treatment plans. This role collaborates and communicates professionally with a multidisciplinary team including oncologists, radiation oncology, nutrition, social work, and spiritual care to provide comprehensive care to medical oncology patients. The navigator acts as a liaison between patients, families, caregivers, and providers to optimize patient outcomes, and serves as a point of contact between inpatient and outpatient oncology patients to facilitate timely discharge, appropriate outpatient follow-up, pre-treatment education, and acts as a liaison for oncologists and hospitalists/in-house providers. The position also establishes and coordinates a supportive care plan, and development of survivorship care plans as recommended by the Commission on Cancer. Active participation in palliative care and multidisciplinary team meetings is expected, functioning as the outpatient clinical representative to identify and refer patients who could benefit from palliative and/or multidisciplinary intervention(s). Patient/family education regarding chemotherapy/immunotherapy is provided, and the role works in collaboration with the authorization and billing team, Resource nurse, and pharmacy to ensure prior authorization and delivery of chemotherapy agents. The navigator works closely with concurrent patients and the multidisciplinary team to help streamline the treatment process, appointments, and address obstacles. The role assesses the physical and psychosocial status of clients through interview, health history, physical examination, and diagnostic studies, recognizing deviations from normal and collaborating with a physician in formulating treatment plans. Objective and accurate documentation of patient care in the electronic health record is required. Relationships are established with professional and/or other health-related groups within the community, and clinical expertise is shared with professional and supportive personnel. Adherence to organizational policies and standard operating procedures, including attendance guidelines, is mandatory. Collaboration with the oncology leadership team to compose, update, and/or edit department-based policies and procedures is also a responsibility. The role collaborates in organizing community oncology events with the oncology team and leadership to support cancer care awareness and programs. Demonstrates professionalism, accountability, and positive leadership qualities, and assumes professional responsibility for current practice and professional growth.
