About The Position

The Oncology Nurse Navigator provides expert individualized care coordination to patients and their families, ensuring a timely schedule for diagnostic work-up, results reporting, and communication between the patient and the multidisciplinary team. This role ensures the patient receives necessary referrals and follow-up. The Oncology Nurse Navigator serves as an advocate for the patient, promoting timely, competent, compassionate care and enhances the physician/patient relationship, improving the patient's experience with cancer. This role involves interacting with co-workers, visitors, and other staff consistent with the values of Jefferson. It requires collaborating and continuously partnering with chronically ill or high-risk patients and their family/caregiver(s), clinic/hospital/specialty providers and staff, and community resources in a team approach. The role also involves evaluating the needs of cancer patients, educating them about their cancer diagnosis, and preparing them and their caregivers for what to expect throughout the cancer care continuum. The Navigator creates and promotes adherence to a care plan developed in coordination with the patient, PCP, oncologist, and family/caregiver(s). Additionally, the role provides clinical support to Patient Navigators and Sidney Kimmel Cancer Center (SKCC) patients, educates patients and families about disease process and potential side effects, and assists patients with treatment decision-making using decision aids as appropriate. The Navigator attends and supports Tumor Boards, aims to reduce emergency room utilization and hospital readmissions through outpatient symptom management, and follows patients throughout the care continuum, including inpatient admissions in collaboration with inpatient care management resources. Support for transitions of care and coordination between SKCC sites is also a key function. The role provides clinical consultation to Patient Navigators and educates multidisciplinary colleagues about the role of Nurse and Patient Navigators, identified barriers, available resources, and the navigation process. Finally, the Navigator supports program growth and patient experience in all patient interactions.

Requirements

  • Graduate of an accredited school of nursing required.
  • Three (3) years of Healthcare/Medical experience.
  • Experience in Oncology/Cancer Care preferred.

  • BSN or completion of BSN within 3 years of hire strongly preferred.
