This position involves planning, developing, and providing diet interventions, medical nutrition therapy, nutrition care, and preventive counseling to individuals and groups in various settings including primary and specialty patient/resident care, acute care, skilled nursing, out-patient, and/or community settings. The role includes providing medical nutrition therapy services and monitoring the nutritional care of clients/patients/residents of all ages, from neonatal to geriatric. This encompasses determining nutritional status through assessment, creating individualized care plans, making provider order recommendations, implementing nutrition therapy (including verbal orders, diet modifications, nutrition support, and oral nutrition supplements), educating and counseling for disease and nutrition risk management, and evaluating the outcomes of interventions. Additionally, it involves optimizing self-management education and skills for chronic conditions, recognizing and providing for nutritional needs based on cultural, ethnic, and religious beliefs, evaluating treatment effectiveness using an evidence-based approach, and acting as a leader for the healthcare team in implementing nutrition therapies. The role also includes educating on diets according to disease state, medical condition, and health risk, incorporating new trends and techniques, assisting in tailoring clinical education materials, performing quality control, maintaining infection control standards, being aware of insurance coverage and reimbursement, and participating in clinical performance improvement activities and research.
