About The Position

The On-Site Specialist Liaison will be responsible for preparing for upcoming on-site specialist visits based on the requirements for that visit, and all follow up with both the on-site provider (specialist) and the referring provider. This position reports directly to the Health Information Management (HIM) Manager.

Requirements

  • Minimum High school diploma or GED required
  • At least 1 years experience working with the frail elderly population.
  • 1 year of related work preferred or OJT for ≥2 years in a medical office setting.
  • High School Diploma or GED
  • Certification/registration as a Medical Office Assistant, RMA, CMA, or CCMA, CNA or LPN required.
  • MA, CMA, RMA, CCMA, CNA or LPN
  • Must be able to work effectively in a team environment.
  • Must possess excellent verbal, written and interpersonal communication skills, organizational, problem solving, attention to detail and critical thinking skills.
  • Must be able to work with changing priorities and interact with individuals of all cultures and levels of authority.
  • Must have a solid understanding of the importance of confidentiality.
  • Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact

Nice To Haves

Responsibilities

  • Monitors Service Tracking for all on-site referrals and develops schedules for each on-site provider visit.
  • Prepares all documentation and sends it to the appropriate on-site provider in the expected timeframe.
  • Prepares space (if required by on-site provider) in the day center/clinic prior to their visit.
  • Communicates with the participant and/or caregiver to inform them of their upcoming on-site appointment one month, one week and one day prior to the appointment.
  • Obtains signed consent forms (if required) from participants, uploads them to the participant’s chart in Elation, and sends to appropriate on-site provider.
  • Follows the on-site standard operating procedure to schedule on-site appointments [Appropriately documents in Service Tracking; Updates the authorization; Adds the appointment to the CareHub calendar; & Requests transportation (if required)].
  • Provides on-site providers with same day notes and obtains them from on-site providers following the visit. Uploads same day notes to participant’s charts in Elation and routes to Primary Care Provider (PCP) for review.
  • Monitors Service Tracking for referrals in “needs report attached” status and either extracts visit notes from external Electronic Medical Record (EMR) or requests notes from on-site provider.
  • Uploads visit notes to Elation and routes to PCP for review in expected timeframe.
  • Reschedules missed on-site visits.
  • Communicates with on-site providers and schedules the monthly on-site visits (Kintegra Dental).
  • Updates the Senior TLC On-Site Calendar in SharePoint.
  • Supports Senior TLC’s mission to encourage and support the quality of life of seniors wishing to continue living in the community; its vision to be the preferred provider of individualized care for seniors in the community; and its values of respect, integrity, accountability, compatible goals, and compassionate care.
  • Other duties as assigned.
