About The Position

For over 125 years, Calvary Hospital and Calvary @Home located in the Bronx, New York has been dedicated to the palliative care of advanced cancer and other terminally ill patients and their families. Through a caring, compassionate, philosophy of non-abandonment, the hospice provides a full spectrum of palliative services, including emotional, spiritual, and care. The hospice receives patients from referring institutions throughout New York City and the entire tristate area. Requirements

• Excellent interpersonal, and communication skills.
• Position requires ability to commute to multiple territories to ensure continuity of care.
• A valid driver’s license is required.
• A vehicle is required to commute to patients. (This requirement does not apply to positions assigned exclusively to Manhattan territory)
• Exhibits an ability and attitude relative to the spirit of caring and tradition at Calvary Hospital as required by specific job duties and functions.
• MSW from an accredited school of social work and one year related experience.
• New York LMSW or LCSW eligible.

Nice To Haves

• Minimum one (1) year bereavement counseling experience is preferred.
• At least one year of Hospice or Home Care experience preferred.

Responsibilities

• The Hospice Social Worker provides medical social services, psychosocial and bereavement counseling to the hospice Patients and families.
