About The Position

Our Hospice team has an immediate opening for a part-time/on-call licensed clinical social worker to support the admissions and psycho-social needs of end-of-life patients and their families on weekends. This role involves providing social work services to clients and families in their place of residence, including evaluation, psychosocial assessment, and reassessment of emotional factors impacting coping. The clinician will formulate and implement care plans, provide crisis intervention, attend Hospice admissions with the Admissions Nurse, and coordinate intake documentation. Additionally, the role includes assisting families with identifying and accessing community/agency resources and making appropriate referrals. All relevant information must be entered into the EHR. The schedule will be available every other Saturday and Sunday from 8:30 am to 5:00 pm.

Requirements

Be adept at working independently within a highly collaborative social work team.
An active State of Maryland LMSW is required.
A minimum of one year of experience in a hospice program or related experience with geriatric population.
A valid Driver’s License.
Reliable transportation to make visits within Montgomery County.

Nice To Haves

An LCSW-C is preferred.

Responsibilities

Attend Hospice admissions with Admissions Nurse, coordinating accompanying intake documentation.
