About The Position

Luminos Hospice is seeking a skilled and compassionate On-Call RN to provide quality care to patients during off-hours and weekends. The role administers nursing care to patients of all ages in their place of residence and coordinates care with the Interdisciplinary Group/Team, patients, families, and referring agencies. The RN assumes responsibility for and participates in coordinating care. The schedule is 7 days on and 7 days off.

Requirements

• Current RN license in the state of practice.
• Current driver’s license.
• Current CPR certification.
• Two years of experience in a clinical care setting.
• One year of full-time hospice experience.
• Nursing skills consistent with generally accepted standards of practice.
• Knowledge of state and federal regulations.
• Ability to work independently and make sound decisions.
• Strong attention to detail and organizational habits.
• Excellent verbal and written communication skills.
• Good interpersonal skills.
• Reliable transportation and valid auto liability insurance.

Responsibilities

• Provide nursing care to patients of all ages in their place of residence.
• Provide quality care during off-hours and weekends.
• Coordinate care with the Interdisciplinary Group/Team, patients, families, and referring agencies.
• Assume responsibility for and participate in the coordination of care.
