On-Call RN Case Manager (7-on-7-off)
About The Position
The On-Call RN Case Manager at Lifted Hospice plays a vital role, combining clinical excellence with genuine compassion to provide care that honors life and brings comfort when it matters most. This role involves delivering personalized nursing care in the patient’s home or care setting, collaborating with an interdisciplinary team to build and follow care plans, and educating and equipping families to help them understand what to expect and how to provide comfort. The position requires leading the bedside experience, managing symptoms, guiding care, and helping families feel confident and supported during end-of-life.
Requirements
- Graduate of an accredited Diploma, Associate, or Bachelor’s nursing program, with an active state license and clinical skills that meet current standards of nursing practice.
- Experience as a registered nurse in a clinical care setting.
- Must read, write and comprehend English.
- Current driver’s license, reliable transportation, and valid auto liability insurance.
Nice To Haves
- Hospice experience preferred.
- Excellent interpersonal skills.
Responsibilities
- Completing thorough assessments to guide care based on each patient’s physical, emotional, and clinical needs.
- Delivering skilled hospice care to help manage pain and improve quality of life.
- Collaborating with the full care team, the patient, and their loved ones to create and follow a unified care plan.
- Documenting care and updates accurately in line with all standards and requirements.
- Educating and encouraging caregivers, offering emotional support, and helping families feel prepared and confident.
