About The Position

The On-Call Registered Nurse provides after-hours triage, clinical guidance, and urgent in-person visits as needed for hospice patients receiving care in home or facility settings, supporting comfort, safety, and continuity of care. This is a dedicated on-call role with a rotating 7 days on / 7 days off schedule, covering hours from 4:30 PM to 8:30 AM. The role is mostly remote with field support as needed, and the assigned coverage area is determined by branch needs and patient census. The position offers a predictable schedule and strong clinical and leadership support.

Requirements

• Licensed or registered in the state in which he/she currently practices.
• Must pass a criminal background check & MVR check.
• Completed health statement acknowledging ability to perform the duties of the position.
• Valid state driver’s license.
• Proof of current automobile insurance.
• Current CPR card; On-line CPR certification accepted; Hands-on CPR certification preferred.
• TB testing per agency policy; (1 or 2 step TB skin test within 12 months of hire & annual TB symptom screening thereafter).
• Ability to work independently, make accurate, and at times, quick judgments.
• Ability to supervise others appropriately.
• Ability to respond appropriately to crisis outside of a hospital setting.
• Acceptance of and adaptability to different social, racial, cultural and religious modes.
• Completes Hospice training program.
• Graduate from an accredited school of nursing
• Currently licensed as a registered nurse to practice in the state.

Nice To Haves

• Active patient contact within the past three years, preferred.
• Minimum 2 years of experience as a registered nurse, preferred.

Responsibilities

• Triage patient and caregiver calls after hours
• Provide clinical guidance and symptom management
• Complete urgent in-person visits as needed
• Coordinate care with interdisciplinary team members
• Document all assessments, interventions, and communication accurately and timely in the EMR HCHB (Homecare Homebase)
