About The Position

At Brighton/Suncrest Hospice, our goal is to change the expectation of hospice care by providing exceptional care and service to our patients. This is achieved by allocating resources to increase staff to patient ratios, thereby increasing clinical visits while lowering clinician caseloads. We are a CHAP certified hospice committed to providing the highest quality of care to patients and their families. The Registered Nurse (RN) will be responsible for providing care and services for a group of hospice patients, visiting patients where they reside, which could include private homes, assisted living facilities, and hospitals. The RN will complete hospice admissions and patient visits as needed, answer calls from patients, families, and customers, and make nursing visits for symptom management, deaths, and admissions. This RN must be passionate about hospice care and be a great advocate for both hospice services and Suncrest Hospice. This position will be after hours on-call working seven days/nights on and seven nights/days off.

Requirements

• Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice.
• One year of experience under license.
• Great clinical and communication skills.
• Ability to work independently and on your own from a day-to-day basis.
• Understanding of hospice rules and regulations.
• Time management skills.
• Ability to learn an electronic medical records system.
• Willingness to drive with reliable transportation, valid driver’s license, and auto insurance to travel across the service area.

Responsibilities

• Providing care and services for a group of hospice patients.
• Visiting patients in private homes, assisted living facilities, and hospitals.
• Completing hospice admissions and patient visits as needed.
• Answering calls from patients, families, and customers.
• Making nursing visits for symptom management, deaths, and admissions.
• Advocating for hospice services and Suncrest Hospice.
• Working after hours on-call.
• Collaborating daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Managing visits, documentation, and meetings.
• Responding to urgent needs by prioritizing and triaging multiple concerns.
