About The Position

Luminos Hospice is seeking a skilled and compassionate On-Call Nurse to join our team. As an On-Call Nurse, you will be responsible for providing quality care to our patients during off-hours and weekends. Primary functions are to administer nursing care for patients of all ages in their place of residence and to coordinate care with the Interdisciplinary Group/Team, patients, families, and referring agency. Assumes the responsibility for, and participates in, the coordination of care. Flexibility to work scheduled on-call shifts, including weekends and holidays.

Requirements

Current state RN or LPN license.
Current State license in the state of practice.
Current driver’s license.
Current CPR Certification.
Two (2) years' experience in a clinical care setting.
One (1) year full-time hospice.
Nursing skills as defined as generally accepted standards of practice.
Knowledge of state and federal regulations.
Ability to work independently and make sound decisions.
Strong attention to detail and organizational habits.
Excellent verbal and written communication skills.
Good interpersonal skills.
Reliable transportation and valid auto liability insurance.

Responsibilities

Administer nursing care for patients of all ages in their place of residence.
Coordinate care with the Interdisciplinary Group/Team, patients, families, and referring agency.
