About The Position

We are seeking a highly skilled and compassionate Licensed Vocational Nurse to join our Home Health and Hospice team in Fresno, CA. As an On-Call LVN for Home Health and Hospice, you will provide clinical support, patient assessment, and triage for home health and hospice patients during after-hours, weekends, and other designated on-call periods.

Requirements

• Active and unrestricted LVN license in the state of California
• Minimum of 1 year of recent experience in Home Health or related field preferred
• Current CPR certification
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Strong clinical assessment, patient triage, and critical-thinking skills
• Ability to recognize changes in patient condition and respond appropriately
• Ability to work independently, prioritize competing needs, make sound clinical decisions, and remain calm and responsive in urgent or high-stress situations
• Excellent organization, time management, problem-solving, documentation, and electronic medical record (EMR) skills
• Compassionate, patient-centered approach
• Flexibility and dependability for after-hours and weekend on-call coverage

Responsibilities

• Answer all after hours and weekend phone calls.
• Triage patient needs - If issue cannot be resolved via phone/facetime then a skilled visit will need to be performed for Home Health calls. Most hospice issues/incidents will need a skilled visit.
• Coverage area will be the Fresno and Visalia Area Territories.
• Updating on-call chat and patient charts as needed.
• Communicate with physicians/RNs/care teams.
