About The Position
We’re hiring a Weekend On-Call Hospice Case Manager (RN) who wants the flexibility of a weekend-only role while providing compassionate end-of-life care to hospice patients and their families. You’ll provide on-call nursing support to hospice patients in homes and care facilities throughout Easton, Lakeville, Taunton, and Rockland. You’ll respond to patient and family needs, complete visits and admissions as needed, and collaborate with the interdisciplinary hospice team to ensure continuity of care.
Requirements
- Active Massachusetts RN license
- At least 1 year of nursing experience
- Comfortable responding independently to patient needs in the field
- Comfortable completing hospice visits and admissions as needed
- Effective communication with patients, families, facilities, and the interdisciplinary team
Nice To Haves
- Hospice, home health, ICU, or oncology experience
Responsibilities
- Provide on-call nursing support to hospice patients in homes and care facilities.

- Assess changes in condition and provide symptom-management support.
- Travel throughout Easton, Lakeville, Taunton, and Rockland.
