About The Position

Blue Stone Therapy is seeking an Occupational Therapy Assistant (COTA) to provide rehabilitation services to adult and geriatric patients at [Facility Name] in [City, State]. This role focuses on delivering skilled occupational therapy services to adult and geriatric patients in a skilled nursing and post-acute rehabilitation setting. The Occupational Therapy Assistant works under the supervision of a licensed Occupational Therapist to implement treatment plans designed to improve activities of daily living (ADLs), functional mobility, strength, cognition, and overall independence.

Requirements

  • Graduate of an accredited Occupational Therapy Assistant program
  • Current Occupational Therapy Assistant license (or eligibility for licensure) in applicable state
  • Strong communication, organization, and clinical reasoning skills
  • Commitment to patient-centered care and professional standards

Responsibilities

  • Provide skilled occupational therapy treatment in accordance with established plans of care
  • Implement individualized treatment programs focused on functional and measurable outcomes
  • Treat adult and geriatric patients in a skilled nursing facility (SNF) setting and rehabilitation settings
  • Monitor patient response to treatment and communicate progress with the supervising Occupational Therapist
  • Maintain timely, accurate documentation to support quality care and reimbursement standards
  • Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes
