About The Position

Empath Home Health is seeking a compassionate PRN Certified Occupational Therapy Assistant (COTA) to provide individualized care to patients in their homes in Ft. Myers, Florida, including Bonita, Estero, and South Ft. Myers. Empath Home Health provides skilled nursing and therapy services to help patients recover and maintain independence at home, along with personal care support as part of a connected continuum of care. The COTA provides occupational therapy services under the supervision of the Occupational Therapist, following the plan of care. The role applies therapeutic self-care and creative activities to improve patients’ independence and physical and mental functioning, evaluates equipment needs, documents services and patient progress, and collaborates with the interdisciplinary team. The schedule is PRN, with 5–15 or more home health visits per week.

Requirements

Active Florida COTA license.
Proof of completion of an Occupational Therapy Assistant program.
At least one year of experience as a licensed Occupational Therapy Assistant.

Responsibilities

Provide occupational therapy services according to the plan of care and under the supervision of the Occupational Therapist.
Apply therapeutic self-care and creative activities to improve patients’ independence and physical and mental functioning.
Evaluate equipment needs to increase patients’ functional levels.
Prepare clinical and progress notes documenting services provided, patient activities, and condition, and submit them in a timely manner.
Collaborate with the interdisciplinary team to ensure seamless, holistic care.
