About The Position
Hometown Home Health is a full-service home health care agency providing skilled medical home care services and clinical nursing care in the Greater Jacksonville and St. Augustine areas. The agency is seeking a Florida-licensed Occupational Therapy Assistant who prioritizes patient care. The role provides occupational therapy services and assessments to patients in their homes, guides patients through prescribed therapeutic care, assists with developing and implementing patient care plans, and prepares progress notes for patient charts after visits.
Requirements
- Graduation after successful completion of an Occupational Therapy Assistant education program accredited by ACOTE of AOTA, or successor organizations.
- Eligibility to take, or successful completion of, the entry-level Occupational Therapy Assistant certification examination developed and administered by NBCOT.
- Licensed to practice in the state of Florida.
- CPR certification; online certification is not accepted.
- At least one year of institutional experience.
- Reliable transportation.

- Membership in the American Occupational Therapy Association is preferred.
Responsibilities
- Provide occupational therapy services and assessments to patients in their homes.
- Guide and instruct patients through prescribed therapeutic care.
- Assist in the implementation and development of patient care plans.
- Prepare progress notes following visits for patients’ charts.
