About The Position

Innovate Rehab and Wellness is a therapist- and veteran-owned skilled healthcare provider with over 25 years of experience, focused on industry-leading, patient-centered care. They offer one-on-one care with minimal group/concurrent treatments and a realistic scheduling approach for a better work environment and outcomes. This Certified Occupational Therapy Assistant (COTA) role is situated in a skilled nursing facility within a collaborative, interdisciplinary team environment.

Requirements

• Associate’s degree from an accredited OTA program
• Current NBCOT certification
• Valid state license as an Occupational Therapy Assistant
• Strong communication and interpersonal skills
• Able to work independently and as part of a team
• Committed to patient-centered care and rehab excellence

Nice To Haves

• SNF or long-term care experience preferred

Responsibilities

• Implement treatment plans created by the Occupational Therapist
• Provide direct patient care per state and facility guidelines
• Monitor and document resident progress; report changes to the OT
• Educate residents and caregivers on adaptive techniques and equipment
• Collaborate with nursing, PT, SLP, and other healthcare staff
• Maintain accurate, timely documentation in the EMR
• Participate in care planning and discharge planning
• Ensure compliance with HIPAA, CMS, and all regulations
