About The Position

The Occupational Therapy Assistant (COTA) will support the delivery of high-quality OT services to help residents regain independence and improve their quality of life. This role involves providing therapeutic interventions under the supervision of an Occupational Therapist (OT) within a skilled nursing facility setting. The COTA will implement treatment plans, provide direct patient care, monitor resident progress, and collaborate with other healthcare professionals. Accurate documentation in the EMR and participation in care planning are key responsibilities. The position is full-time, Monday through Friday with flexible scheduling, and offers a competitive hourly rate based on experience.

Requirements

• Associate’s degree from an accredited OTA program
• Current NBCOT certification
• Valid state license as an Occupational Therapy Assistant
• Strong communication and interpersonal skills
• Able to work independently and as part of a team
• Committed to patient-centered care and rehab excellence

Nice To Haves

• SNF or long-term care experience preferred

Responsibilities

• Implement treatment plans created by the Occupational Therapist
• Provide direct patient care per state and facility guidelines
• Monitor and document resident progress; report changes to the OT
• Educate residents and caregivers on adaptive techniques and equipment
• Collaborate with nursing, PT, SLP, and other healthcare staff
• Maintain accurate, timely documentation in the EMR
• Participate in care planning and discharge planning
• Ensure compliance with HIPAA, CMS, and all regulations
