FeldCare Connects, an app-based therapy network, is seeking a self-motivated Certified Occupational Therapy Assistant (COTA) to provide quality home healthcare to patients of all ages in San Dimas. Requirements

• Graduation from an accredited Certified Occupational Therapy Assistant program.
• California Certified Occupational Therapy Assistant license.
• Ability and enthusiasm in working with all ages.

Nice To Haves

• Bilingual is a plus.

Responsibilities

• Provide patient care related to functional status, activities of daily living, fine motor coordination, home assessments, and adaptive equipment.
• Follow the treatment plan established by the Occupational Therapist (OT).
• Perform interventions to improve and enhance patient well-being.
• Evaluate patient progress.
• Provide services as defined by state laws governing COTA practice, in accordance with the plan of care, and in coordination with the healthcare team.
• Assist patients in using therapeutic, creative, and self-care activities to improve functioning and safety.
• Observe, record, and report patient response to treatment and changes in condition.
• Instruct patients, families, caregivers, and the healthcare team on how they can assist the patient in therapy.
