About The Position

Innovate Rehab and Wellness is seeking a Licensed Certified Occupational Therapy Assistant (COTA) for a Full-Time position in a Skilled Nursing Facility in Winnfield, LA. This role involves supporting the delivery of high-quality Occupational Therapy services to help residents regain independence and improve their quality of life. The position offers a Monday-Friday flexible schedule and a competitive hourly rate based on experience. Requirements

• Associate’s degree from an accredited OTA program
• Current NBCOT certification
• Valid state license as an Occupational Therapy Assistant
• Strong communication and interpersonal skills
• Able to work independently and as part of a team
• Committed to patient-centered care and rehab excellence

Nice To Haves

• SNF or long-term care experience preferred

Responsibilities

• Implement treatment plans created by the Occupational Therapist
• Provide direct patient care per state and facility guidelines
• Monitor and document resident progress; report changes to the OT
• Educate residents and caregivers on adaptive techniques and equipment
• Collaborate with nursing, PT, SLP, and other healthcare staff
• Maintain accurate, timely documentation in the EMR
• Participate in care planning and discharge planning
• Ensure compliance with HIPAA, CMS, and all regulations
