You’ll provide home-based therapy as directed by the attending physician and follow treatment plans and goals established by the Occupational Therapist. Requirements

  • Occupational Therapy Assistant (OTA) with a current license in the state of employment
  • Minimum of one year of experience as an OTA in an acute care setting (required)
  • Valid driver’s license and auto insurance (must be listed driver)
  • Ability to meet all physical demands of the role

Nice To Haves

  • Home care experience preferred

Responsibilities

  • Plan of Care Provide skilled occupational therapy as ordered by the attending physician
  • Monitor patient conditions and report changes to the Occupational Therapist
  • Minimize Patient Risk Assess functional limitations and provide appropriate interventions
  • Work collaboratively with the Occupational Therapist to improve patient outcomes
  • Family Educator / Advocate Educate patients and families on conditions and therapy plans
  • Instruct on proper use of assistive equipment (wheelchairs, walkers, braces, etc.)
  • Policies & Documentation Complete all clinical documentation per agency protocol and Medicare/Federal guidelines
  • Rules & Regulations Follow all agency policies, procedures, and regulatory requirements
  • Communicate schedule or availability changes to appropriate staff
  • Operations Attend in-service trainings and mandatory agency meetings
