About The Position
Hometown Home Health is a full service home health care agency offering a wide range of skilled medical home care services and clinical nursing care in the Greater Jacksonville and St. Augustine areas. They are searching for a licensed Occupational Therapy Assistant in the state of Florida who prioritizes patient care. Both full-time and PRN roles are available.
Requirements
- Graduated after successful completion of an Occupational Therapy Assistant education program accredited by: The Accreditation Council for Occupational Therapy Education (ACOTE) of the American Occupational Therapy Association, Inc. (AOTA), or its successor organizations.
- Eligible to take, or successfully completed, the entry-level certification examination for Occupational Therapy Assistants developed, and administered, by the National Board for Certification in Occupational Therapy, Inc. (NBCOT)
- Licensed to practice within the state of Florida
- CPR certification required (Online certification not accepted)
- At least one year of institutional experience
- Reliable Transportation

- Member of the American Occupational Therapy Association preferred
Responsibilities
- Provide occupational therapy services and assessments to patients in their homes
- Guide and instruct patients through prescribed therapeutic care
- Assist in the implementation and development of patient care plans
- Prepare progress notes following visits for patients’ charts
