About The Position

Aspire Home Healthcare is hiring a full-time Occupational Therapist to serve the Yuba City, Grass Valley, and Oroville areas, with travel to Grass Valley and Oroville as needed. This salaried home-health position starts at $125,000 annually. The Occupational Therapist evaluates and treats patients to improve their ability to perform daily living and work activities. The role involves designing and implementing individualized therapy plans to help patients regain independence at home and coordinating care with the broader rehabilitation team.

Requirements

• Licensed Occupational Therapist (OT) in California.
• Current physical examination from within the last 6 months.
• Current TB test from within the last 12 months and CPR certification.
• Valid driver’s license and reliable transportation.
• Ability to attend a 2–3 day orientation at the Yuba City office.

Nice To Haves

• At least 1 year of OT experience.
• Experience with OASIS and WellSky (Kinnser).

Responsibilities

• Provide effective patient care using approved therapy techniques and procedures.
• Evaluate patients’ condition, functional level, and treatment goals.
• Develop individualized treatment plans and update them as needed.
• Supervise COTAs and HHAs according to care plan requirements.
• Document treatment notes and progress in the patient’s chart daily.
• Assist patients and families with home exercise programs and adaptive equipment needs.
• Recommend and evaluate adaptive equipment for the home setting.
• Maintain compliance with HIPAA regulations and company policies.
• Coordinate total patient care with the healthcare team.
• Complete discharge summaries and obtain orders when needed.
• Submit patient documentation and visit records in a timely manner.
