About The Position

Innovate Rehab and Wellness is seeking a Licensed Occupational Therapist (OT) for a Full-Time position in a Skilled Nursing Facility setting in Kansas City, MO. The schedule is Monday – Friday with flexible hours. We are a therapist- and veteran-owned company with over 25 years of experience providing skilled healthcare services, focused on industry-leading, patient-centered care within a supportive, collaborative team environment. We offer a one-on-one therapy focus, with group/concurrent treatments being less than 1%, and realistic schedules to promote work-life balance. There is a strong emphasis on clinical outcomes and job satisfaction.

Requirements

• Master's or Doctorate in Occupational Therapy (accredited program)
• Valid state OT license
• Strong communication and problem-solving skills
• Passion for patient-centered care
• CPR certification preferred

Nice To Haves

• Experience as an OT preferred (new grads welcome)

Responsibilities

• Evaluate patients’ functional abilities and therapy needs
• Create and deliver individualized treatment plans
• Help patients improve motor skills, coordination, and daily function
• Support patients in regaining independence and quality of life
• Conduct thorough OT evaluations
• Develop and adjust personalized treatment plans
• Use therapeutic techniques and adaptive equipment
• Train patients and caregivers on exercises and assistive devices
• Educate on strategies to improve function and safety
• Collaborate with PT, SLP, physicians, and the full care team
• Document patient progress and treatment changes accurately
• Stay current with OT best practices and research
