About The Position

Full-Time Occupational Therapist (OT) – Home Health. Aspire Home Healthcare has been putting patients first for 26 years. We're currently seeking a full-time Occupational Therapist to provide in-home care to patients in Delaware, OH and nearby communities. As an Occupational Therapist (OT), you'll evaluate patients, develop and implement individualized treatment plans, and collaborate with our rehab team to help patients regain independence and improve their quality of life in a home health setting.

Requirements

• Active OT license in Ohio
• Current TB test and CPR certification
• Valid driver’s license and auto insurance
• Ability to attend orientation in Dublin, OH

Nice To Haves

• 1+ year of Occupational Therapy experience
• Home health experience
• OASIS and WellSky/Kinser experience

Responsibilities

• Evaluate patient condition and functional abilities
• Develop and implement personalized treatment plans
• Focus on ADLs (activities of daily living) and functional independence
• Recommend and train patients on adaptive equipment and home modifications
• Supervise COTAs and HHAs per plan of care
• Maintain accurate and timely documentation
• Educate patients and families on home programs and safety
• Submit notes and visit documentation promptly
• Adhere to HIPAA confidentiality standards
