The Occupational Therapist is responsible and accountable for residents’ evaluation, ongoing assessment, planning, and implementation of therapy interventions. The Occupational Therapist develops and revises plan of care based on the residents’ needs and establishes functional goals, outcomes, and discharge plan. They plan for residents’ discharge and anticipate for long term and/or short term residents’ needs. They also educate other health care team members, residents, family, and caregivers on information regarding the care plan for the residents.
