About The Position

Innovate Rehab and Wellness is seeking a licensed Occupational Therapist to provide patient-centered care in a skilled nursing facility. The therapist evaluates patients’ functional abilities and therapy needs, creates and delivers individualized treatment plans, and helps patients improve motor skills, coordination, and daily function so they can regain independence and quality of life. The role focuses on one-on-one therapy; group and concurrent treatments make up less than 1% of treatment. The company describes its schedules as realistic and emphasizes clinical outcomes, job satisfaction, and a supportive, collaborative team environment. The schedule is Monday through Friday and flexible, with a competitive hourly rate based on experience. Full-time benefits begin on the first day of employment.

Requirements

Master’s or doctorate in Occupational Therapy from an accredited program.
Valid state occupational therapy license.
Strong communication and problem-solving skills.
Passion for patient-centered care.

Nice To Haves

Occupational therapy experience; new graduates are welcome.
CPR certification.

Responsibilities

Conduct thorough occupational therapy evaluations.
Develop and adjust personalized treatment plans.
Use therapeutic techniques and adaptive equipment.
Train patients and caregivers on exercises and assistive devices.
Educate patients on strategies to improve function and safety.
Collaborate with physical therapists, speech-language pathologists, physicians, and the full care team.
Accurately document patient progress and treatment changes.
Stay current with occupational therapy best practices and research.
