Occupational Therapist PRN - Daniel Island, SC
About The Position
FOX Rehabilitation is expanding in Daniel Island, SC, and is seeking dedicated, licensed Occupational Therapists to provide exceptional one-on-one care to older adults in their homes or within Senior Living Communities. The role focuses on improving patient mobility, independence, and quality of life through the Geriatric House Calls™ model. This position offers autonomy, flexibility, and the opportunity to create a personalized treatment schedule. Clinicians can also integrate telehealth and virtual care for enhanced flexibility and patient support. This is a high-impact OT role aimed at positively influencing the lives of community members.
Requirements
- Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Occupational Therapy program
- Basic computer and EMR proficiency
- Current CPR certification
- Exerting medium physical effort, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly
- Performing CPR
- Providing moderate to maximum assistance with patient transfers and bed mobility
- Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities
- Driving to patient homes and senior living communities as part of the Geriatric House Calls™ model
- Wearing PPE when required

Responsibilities
- Provide one-on-one Occupational therapy to older adults wherever they call home (Private residences, Senior Living Communities, Independent Living, Assisted Living, Memory Care)
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
- Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence
- Document effectively within FOX’s EMR system in accordance with regulatory standards
- Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place
- Provide telehealth / virtual care when appropriate to extend access and flexibility
