About The Position

Tattnall Health Center is seeking compassionate and motivated Occupational Therapists (OT) to join our rehabilitation team in both Full Time and Per Diem capacities. The Occupational Therapist will evaluate, develop, and implement individualized treatment programs designed to help residents achieve maximum independence, improve functional performance, enhance safety, and maintain the highest possible quality of life. Requirements

  • Current Georgia Occupational Therapist (OT) license in good standing
  • Graduate of an accredited Occupational Therapy program
  • Current registration with the National Board for Certification in Occupational Therapy (NBCOT) preferred
  • Previous experience in skilled nursing, rehabilitation, or long-term care preferred
  • Strong clinical, communication, and interpersonal skills
  • Ability to work independently and as part of a multidisciplinary team
  • CPR certification preferred

Responsibilities

  • Evaluate residents and develop individualized occupational therapy treatment plans based on clinical findings and functional goals
  • Provide skilled occupational therapy interventions to improve activities of daily living (ADLs), instrumental activities of daily living (IADLs), upper extremity function, cognition, coordination, balance, and safety awareness
  • Establish realistic treatment goals and modify treatment plans as needed based on resident progress
  • Assess and recommend adaptive equipment, assistive devices, and environmental modifications to enhance independence and safety
  • Monitor resident responses to treatment and communicate progress, concerns, and recommendations to the interdisciplinary care team
  • Accurately document evaluations, treatment sessions, progress notes, recertifications, and discharge summaries in accordance with facility, state, and federal guidelines
  • Educate residents, families, and caregivers regarding treatment plans, home programs, adaptive techniques, and safety strategies
  • Collaborate with physicians, nursing staff, therapy team members, and other healthcare professionals to coordinate resident care
  • Supervise and provide clinical direction to Certified Occupational Therapy Assistants (COTAs) as required
  • Maintain compliance with all federal, state, professional, and facility regulations and standards
