A Path of Care Home Health is seeking a PRN Occupational Therapist (OT) to join our Tulsa team. Requirements

  • Graduation from an accredited occupational therapy program OR eligibility for the national registration exam OR at least 2 years of OT experience with a satisfactory proficiency exam
  • CPR certification within 60 days of hire
  • Ability to lift up to 50 pounds
  • Friendly, respectful, dependable, and committed to professionalism and teamwork

Responsibilities

  • Evaluating patient functional levels and collaborating with physicians to develop individualized plans of care
  • Preparing clinical notes, progress notes, and documentation in accordance with regulatory standards
  • Providing therapeutic treatments and interventions based on the plan of care
  • Consulting with families and agency staff to support patient goals
  • Participating in in-service programs and interdisciplinary collaboration
  • Assessing equipment needs and ensuring patients receive appropriate assistive devices
  • Reviewing and revising plans of care as needed (at least every 60 days)
  • Practicing infection control and universal precautions
  • Reporting suspected abuse according to state and federal guidelines
  • Planning for patient discharge and evaluating treatment effectiveness
