About The Position

Elara Caring is seeking a compassionate Occupational Therapist to provide home health care. The role evaluates patients’ occupational therapy needs, develops individualized treatment plans, and delivers therapy to support independence, daily living skills, fine motor coordination, and overall function. The Occupational Therapist educates patients and caregivers, monitors progress, communicates with physicians and the care team, coordinates adaptive equipment, and provides guidance to other staff involved in patient care.

Requirements

• Current, unrestricted OT license valid for the state of work.
• Graduate of an accredited Occupational Therapy program.
• Minimum one year of experience in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Valid driver's license and acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations.

Responsibilities

• Evaluate patients to identify occupational therapy needs and develop individualized treatment plans.
• Provide occupational therapy to improve independence, daily living skills, fine motor coordination, and overall function.
• Educate patients and caregivers on home therapy programs, adaptive techniques, and equipment.
• Monitor patient progress and adjust treatment plans based on changing needs and physician orders.
• Communicate patient progress, changes in condition, and safety concerns to physicians and the care team.
• Coordinate adaptive equipment and specialized devices, including custom splints when needed.
• Provide guidance and supervision to COTAs, Home Health Aides, and other clinical staff involved in patient care.
