Requirements

  • Graduate from an accredited program of Occupational Therapy
  • Current licensure as an Occupational Therapist in the State of WI
  • CPR Certification (or ability to obtain within 3 months)

Responsibilities

  • Work closely with patients and patient family members by evaluating and providing diagnostic and therapeutic services.
  • Collaborate with other departments and professionals regarding patient treatment/activity progress.
  • Service as a member of the treatment team involved in the discharge planning with follow up care.
  • Maintain patient records including initial evaluations/assessments, progress notes, and discharge summaries.
