About The Position

The Program of All-Inclusive Care for the Elderly (PACE) offers a variety of services, with many of them provided onsite at a PACE Center. PACE is an alternative to nursing home care and – through an interdisciplinary care team (IDT) of physicians, nurse practitioners, nurses, social workers, therapists, van drivers, and aides – coordinates and provides preventive, primary, acute, and long-term care services, so older individuals can continue living in the community. Senior Care Partners PACE is seeking a compassionate and innovative Occupational Therapist (OT) to join our interdisciplinary team and help participants live as independently and safely as possible in their homes and communities. This is more than a traditional therapy role. You’ll have the opportunity to look at the whole person and their environment —helping participants overcome barriers, adapt to changing abilities, and continue doing the activities that matter most to them. If you’re passionate about person-centered care and enjoy collaborating with a team to create meaningful outcomes, we invite you to bring your expertise to Senior Care Partners PACE. Your work will directly support participants in maintaining or improving their ability to perform activities of daily living (ADLs) and instrumental activities of daily living (IADLs) —from personal care and mobility to household tasks and community living. You’ll combine clinical expertise, creativity, education, adaptive strategies, and environmental modifications to help each participant achieve their individual goals and remain engaged in the community.
