About The Position

The Occupational Therapist delivers occupational therapy services under a physician-approved plan of care in a home health setting. The role is accountable for the safety and effectiveness of service delivery, ensures identified assessment problems are addressed in the plan of care or documents why they are not, completes documentation accurately and on time, and supervises a home health aide when an RN is not active in the case. Other duties may be assigned.

Requirements

• Current occupational therapy license for the state of assigned work.
• Graduate of an occupational therapy program accredited by ACOTE/AOTA or a successor organization.
• Eligible to take, or have successfully completed, the NBCOT entry-level certification examination for occupational therapists.
• One year of related occupational therapy experience; candidates with less than one year require regional clinical leader approval and appropriate mentoring and/or an established residency program.
• Current CPR certification.

Responsibilities

• Provide all aspects of occupational therapy service delivery under a physician-approved plan of care.
• Ensure the plan of care addresses all problems identified in the assessment, or document the rationale for problems not addressed.
• Ensure the safety and effectiveness of the occupational therapy service delivery process.
• Complete documentation accurately and in a timely manner.
• Supervise the home health aide when an RN is not active in the case.
• Perform other duties as assigned.
