About The Position

The Home Care Occupational Therapist provides skilled occupational therapy services directly in patients’ homes. The role is responsible for evaluating functional abilities, identifying barriers to independence, and implementing treatment plans that support safe and effective performance of daily living activities in a home environment.

Requirements

  • Degree in Occupational Therapy from an accredited program (BS, MS, or OTD)
  • Current Occupational Therapist license in the practicing state
  • NBCOT certification (if required by state)
  • Valid driver’s license and reliable transportation
  • Ability to travel regularly within assigned geographic area

Nice To Haves

  • Prior experience in home health, outpatient, or rehabilitation settings
  • Experience with geriatric, neurological, orthopedic, or medically complex populations
  • Familiarity with electronic medical record (EMR) systems
  • Strong independent clinical judgment and time management skills
  • Ability to work independently in a field-based environment
  • Strong communication and patient education skills
  • Sound clinical reasoning and problem-solving ability
  • Adaptability to varied home environments and patient needs

Responsibilities

  • Conduct in-home occupational therapy evaluations and establish patient-centered goals
  • Develop and implement individualized treatment plans to improve functional independence
  • Provide therapy interventions targeting activities of daily living (ADLs) and instrumental activities of daily living (IADLs)
  • Assess home safety and recommend modifications to reduce risk and improve accessibility
  • Train patients and caregivers on adaptive strategies, compensatory techniques, and safe mobility practices
  • Recommend and instruct on use of adaptive equipment and assistive devices
  • Document evaluations, progress notes, and discharge summaries in compliance with regulatory and agency standards
  • Coordinate care with physicians, nurses, physical therapists, and other healthcare professionals
  • Monitor patient progress and adjust interventions as clinically indicated
  • Educate patients and families on injury prevention, fall prevention, and condition management
  • Maintain compliance with Medicare guidelines, state regulations, and agency policies
