Matrix Home Care is seeking a licensed Occupational Therapist (OT) for an immediate PRN opportunity providing home health therapy visits in the North Port and Port Charlotte areas. This is a great opportunity for an OT who is looking for flexible PRN work while making a meaningful difference in patients’ homes. Requirements
- Active Occupational Therapist license in the State of Florida
- Home health experience preferred
- Strong clinical assessment, documentation, and communication skills
- Reliable transportation
- Ability to work independently in the field
- Professional, dependable, and patient-focused approach to care
- Under Florida law, this role is subject to background screening per Chapter 435, Florida Statutes, including fingerprinting and review for eligibility before employment.
Responsibilities
- Provide occupational therapy services to patients in the home setting
- Evaluate patients’ functional abilities, needs, and goals
- Develop and follow individualized treatment plans based on patient needs
- Assist patients with improving daily living skills, safety, mobility, strength, coordination, and independence
- Educate patients and caregivers on therapy goals, home safety, adaptive techniques, and recommended exercises
- Monitor patient progress and communicate any changes or concerns to the care team
- Complete timely and accurate documentation in accordance with company and regulatory requirements
- Collaborate with nurses, therapists, physicians, and other members of the care team to support quality patient outcomes
