About The Position
Concept Rehab is a therapist-owned and operated company with over 45 years of experience providing high-quality care in Post-Acute and Skilled Nursing Facility (SNF) settings. They offer a supportive, growth-focused environment for clinicians. This role involves evaluating patients, developing individualized treatment plans, and providing skilled occupational therapy services to maximize patient independence and function. The position focuses on ADLs, mobility, safety awareness, and adaptive equipment needs. Collaboration with physicians, care teams, residents, and families is essential, as is maintaining accurate documentation, billing, and discharge planning. Educating patients, caregivers, and staff is also a key responsibility.
Requirements
- Degree from an accredited Occupational Therapy program
- Active state Occupational Therapist license
- Strong communication, clinical reasoning, and patient-centered care skills
