About The Position

Signature Healthcare at Home is seeking an experienced Occupational Therapist to serve patients in the Portland region and surrounding communities. The role provides occupational therapy in patients’ homes, helping them improve their ability to manage activities of daily living and functional mobility. The Occupational Therapist evaluates patients’ functional status and home environments, identifies equipment needs, participates in developing plans of care, and establishes treatment programs and goals. The role also involves communicating care plans to referring physicians and other professionals, teaching or retraining daily living skills, fabricating splints, and instructing patients in the use of adaptive equipment. Requirements

• Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Association and the American Occupational Therapy Association.
• Registered with the National Registration Examination of the American Occupational Therapy Association.
• BLS for Healthcare Workers certification from the American Heart Association (AHA).
• Current driver’s license and auto insurance.
• Experienced Occupational Therapist.

Responsibilities

• Evaluate patients’ functional status and home environments for hazards or barriers to independent living.
• Identify patients’ equipment needs.
• Participate in developing the total plan of care.
• Develop treatment programs and establish goals for improved function.
• Communicate plans of care to referring physicians and other professionals.
• Teach new skills or retrain patients in daily activities affected by illness or injury.
• Fabricate splints and instruct patients in the use of adaptive equipment to improve function.
• Contribute to developing best-practice guidelines.
