About The Position

Concierge Home Care is hiring a Home Health Occupational Therapist to serve Fort Walton and Okaloosa County, Florida. The role provides one-to-one patient care in patients’ homes, with territories designed to minimize drive time through clustered scheduling. The position offers a pay-per-visit compensation model, a flexible schedule, and autonomy, with additional earning potential based on productivity.

The therapist evaluates patients, develops individualized treatment plans, supports patients’ safety and functional independence, and educates patients and caregivers. The role includes collaboration with physicians and interdisciplinary teams, supervision of Certified Occupational Therapy Assistants, and completion of clinical documentation using streamlined technology. Structured onboarding, mentorship, preceptorship, training, and clinical support are provided.

Requirements

• Active, unencumbered Florida Occupational Therapist license.
• Valid driver’s license, reliable transportation, and auto insurance.
• CPR certification.
• Ability to supervise COTAs.
• Successful completion of an AHCA Level 2 background screening may be required.

Nice To Haves

• Home health experience is preferred but not required.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
