About The Position

By The Bay Health is seeking a Home Care Occupational Therapist to evaluate and treat patients with impaired functional ability in their place of residence. Following established policies and procedures, the therapist is accountable for coordinating care, implementing physician orders, and providing care from admission through discharge. The role serves adult and geriatric patients and includes assessing patient and equipment needs, collaborating on individualized plans of care, and counseling patients and caregivers. The position is full-time, Monday through Friday, 8:30 a.m. to 5:00 p.m., with a required weekend rotation. The service area is Sonoma County, with coverage in Marin County as needed. By The Bay Health follows CDPH vaccine requirements for healthcare personnel.

Requirements

Bachelor's degree in occupational therapy or equivalent.
Current California license in Occupational Therapy.
BLS certification required.

Nice To Haves

Master's degree in occupational therapy preferred.
One year's experience in a healthcare setting preferred.

Responsibilities

Evaluate adult and geriatric patients and interpret findings to determine the nature and extent of dysfunction.
Collaborate with physicians, patients, and team members to establish, implement, and evaluate plans of care and goals, and periodically revise treatment or care plans.
Obtain required physician orders for all care provided.
Provide comprehensive ongoing assessments of patients and their equipment needs.
Counsel patients and caregivers in meeting patient needs and goals.
Notify the attending physician and other appropriate healthcare professionals and staff members promptly of significant changes in a patient's condition.
Coordinate care, implement physician orders, and provide care from admission through discharge.
