About The Position

Pinnacle Home Care, Florida’s largest Medicare-certified home health provider, is seeking PRN Occupational Therapists (OT) to join their award-winning team. They have been delivering high-quality, patient-centered care for over two decades and are looking for individuals who want to make a difference in patients' lives by providing compassionate healthcare at home. The service area covers Melbourne to Palm Bay/Micco/Sebastian areas, including adjacent beachside.

Requirements

  • Must have a current OT license (state-specific).
  • Graduate of an accredited occupational therapy program recognized by the American Medical Association and the American Occupational Therapy Association.
  • Registered with the National Registration Examination of the American Occupational Therapy Association.
  • Valid BLS certification.
  • Minimum two years of OT experience (home health experience preferred).
  • Excellent communication, organizational, and time management skills.
  • Must have a valid driver’s license and access to a reliable and insured vehicle.
  • Ability to visit homes that may not be wheelchair accessible.
  • Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.
  • Engage in physical activities such as walking, standing, stooping, and occasional lifting or turning of patients.

  • Home health experience preferred.

Responsibilities

  • Assess the patient’s functional status (muscle function, endurance, visual coordination, communication skills, self-care, work capacity, etc.).
  • Evaluate the home environment for hazards or barriers to independent living and identify necessary equipment.
  • Collaborate in developing a comprehensive care plan.
  • Design a treatment program with goals for improved function and communicate the plan to the referring physician and other team members.
  • Teach or retrain patients in daily activities lost due to illness or injury, following agency policies.
  • Fabricate splints and instruct patients on using adaptive equipment to enhance function.
  • Provide training in the use of prosthetic and/or orthotic devices.
  • Maintain accurate clinical records and report any changes in the patient’s condition to the physician.
  • Participate in in-services, case conferences, and meetings to promote coordinated care with healthcare team members.
  • Work alongside physicians and discharge planners to provide health education to patients and families/caregivers.
  • Identify patient and family/caregiver needs for additional services and make appropriate referrals.
  • Confirm daily visit schedules to ensure seamless coordination with other staff.
  • Supervise Certified Occupational Therapy Assistants in accordance with agency policies and state regulations.
