About The Position

VNA Health at Home, part of a faith-based health care network, is seeking an Occupational Therapist to provide skilled therapy services to patients in their homes. The therapist evaluates patients’ abilities and needs, sets goals, and develops plans to help patients achieve independence, improve quality of life, and prevent potential health-related problems. The role includes assessing physical, cognitive, and psychosocial abilities; implementing personalized treatment plans; educating patients and families; documenting care; monitoring progress; and collaborating with the home health team.

Requirements

Bachelor’s degree from an accredited school approved by the American Occupational Therapy Association.
Active registered occupational therapist license in the state of practice.
Obtain and maintain current CPR/BLS certification.
One year of occupational therapy experience in a clinical setting.
Current driver’s license.
Comply with federal, state, and local laws and regulations.

Responsibilities

Assess patients’ physical, cognitive, and psychosocial abilities to determine therapy needs.
Develop and implement personalized treatment plans, including exercises, adaptive techniques, and assistive devices to improve functional abilities.
Teach patients and family members about home safety, adaptive equipment, and strategies for managing daily activities.
Maintain accurate and timely records of patient progress, treatment plans, and modifications to therapy programs in accordance with compliance and regulatory standards.
Monitor patient progress and adjust treatment plans as needs change.
Use outcome-based measures to assess the effectiveness of therapy interventions.
Communicate changes in patients’ conditions and collaborate with physicians, nurses, physical therapists, speech therapists, and other home health team members.
